Position Summary

The Behavioral Health Case Manager (BHCM) is responsible for coordinating and supporting mental health care within Elliot Behavioral Health Services (EBHS). The BHCM provides clinical case management support to high acuity patients across the lifespan (children, adolescents, adults, and geriatrics) and their families. Assigned to a high acuity treatment team within EBHS, the BHCM supports approximately 40 patients, coordinating treatment needs, developing, monitoring, and evaluating treatment plans, and empowering patients to actively participate in their treatment to achieve independence.

The BHCM works with patients to maximize emotional, social, and physical wellbeing and ensures the appropriate and efficient use of healthcare services. The role involves collaboration with a multidisciplinary team, including psychiatrists, nurse practitioners, therapists, primary care providers, and other specialists, to promote patient, family, staff, and physician satisfaction through coordinated and continuous care.

Primary Duties and Responsibilities
Communication
  • Educate and support patient transition to community-based services.
  • Coordinate and respond to patient referrals promptly, acting as a resource for providers.
  • Communicate effectively across diverse populations with professionalism, sensitivity, and tact.
  • This role is primarily remote; employees must utilize all designated communication tools. Home office supplies are not provided.
Information
  • Use proper systems for scheduling and updates.
  • Stay current with mental health needs and social services within New Hampshire.
Operations
  • Monitor mental status, SDOH needs, and case management status of high acuity patients for transition planning.
  • Prepare reports on outcomes, successes, and areas for growth.
  • Develop effective working relationships with the medical team and participate in weekly on-site team meetings. Off-site duties are approved by the supervisor.
Compliance/Regulation
  • Follow policies and procedures of Elliot Health Systems.
  • Adhere to HIPAA and confidentiality laws, including 42CFR regulations.
  • Maintain professional boundaries and follow infection control and safety guidelines.
Professional Development
  • Attend required training and maintain certifications such as CANS/ANSA.
  • Stay updated on policy and procedural changes.
Essential Job Functions
  • Assess crisis situations and respond or refer appropriately.
  • Assist high acuity patients with transition to community providers, including scheduling and intake paperwork.
  • Coordinate and provide safe, timely, effective, and patient-centered care.
  • Administer mental health and substance abuse screenings.
  • Provide education, facilitate referrals, and advocate for patients' clinical decisions.
  • Conduct crisis interventions as needed.
  • Maintain accurate documentation per federal, state, and organizational guidelines.
  • Participate in meetings and community programs as assigned.
