Job summary

Volunteers of America Dakotas is hiring a Case Manager to provide targeted case management helping clients access clinical, behavioral health, and social services to support treatment, recovery, and stability. The role focuses on individuals with serious mental illness and/or substance use disorders within VOA’s CCBHC expansion.

The position requires a bachelor’s degree (or QMHP) and experience in case management and EHR documentation.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or a related field; or QMHP designation under SD Medicaid standards.
• Experience in case management or behavioral health settings is required.
• Preferred: experience with populations with SMI/SUD and with rural veterans/youth.

Responsibilities

• Provide targeted case management coordinating clinical, behavioral health, and social services.
• Navigate Medicaid enrollment/redetermination and connect clients to coverage and care.
• Engage outreach efforts to connect underserved individuals to CCBHC services.
• Maintain timely, accurate EHR documentation for Medicaid/CCBHC reporting.
• Participate in training on evidence-based case management and work with clients with co-occurring conditions.
