Job summary

VOA Dakotas is expanding its same-day outpatient behavioral health capacity to help individuals with serious mental illness and/or substance use disorders. The Case Manager will coordinate clinical and social services, assist with Medicaid enrollment, and connect clients to housing, transportation, and other supports.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or related field; or QMHP designation under SD Medicaid provider standards.
• Experience in case management, social services, or behavioral health settings; EHR documentation experience.
• Preferred: targeted case management in Medicaid behavioral health; experience with SMI, SUD, or co-occurring IDD/TBI.
• Strong organizational, communication, and cultural humility skills for diverse populations.
• Competencies include care coordination, resource navigation, documentation/compliance, advocacy, collaboration, and time management.

Responsibilities

• Care Coordination & Navigation: Provide targeted case management to help clients navigate clinical, behavioral health, and social service systems.
• Benefits & Medicaid Navigation: Assist Medicaid enrollment and redetermination, including new federal work requirements.
• Outreach & Engagement: Support outreach to connect underserved individuals to CCBHC services and promote treatment engagement.
• Documentation & Compliance: Maintain timely, accurate case notes in the EHR per CCBHC/Medicaid requirements.
• Professional Development: Participate in training on evidence-based practices for engagement and recovery.
