Job summary

Volunteers of America, Dakotas is seeking a Case Manager to provide targeted case management, helping clients access behavioral health services, benefits, and community resources to support treatment, recovery, and stability.

You will coordinate care across the CCBHC team, assist with Medicaid enrollment, and ensure timely documentation in the electronic health record, while engaging with veterans, youth, and rural populations to promote engagement and reduce barriers.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, human services, or a related field; QMHP designation or eligibility required.
• Experience in case management, social services, or behavioral health settings; EHR documentation experience.
• Valid driver’s license and reliable transportation.
• Working knowledge of Medicaid eligibility and community resource systems.
• Experience serving veterans, youth, or rural populations is a plus.

Responsibilities

• Provide targeted case management to help clients navigate clinical, behavioral health, and social service systems.
• Coordinate services across the CCBHC care team, including Substance Use Counselors, Mental Health Therapists, medical providers, and DCO partners.
• Develop and monitor individualized case management plans aligned with each client’s treatment and recovery goals.
• Assist clients with Medicaid enrollment and redetermination, including navigation of new federal community engagement (work) requirements.
• Connect uninsured or Medicaid-eligible individuals to coverage and care, in coordination with outreach and engagement efforts.
• Help clients access housing, employment, transportation, and other social determinants of health resources.
• Support outreach and engagement activities to connect underserved individuals, including veterans and youth, to CCBHC services.
• Conduct follow-up contacts to promote treatment engagement and reduce missed appointments.
• Maintain timely, accurate case management documentation in the electronic health record consistent with CCBHC and Medicaid requirements.
• Ensure documentation supports CCBHC quality measure reporting.
• Participate in training on evidence-based case management practices tailored to engagement, treatment, and rehabilitation for individuals with SMI/SUD.
• Build skills to support clients with co-occurring conditions, including SUD intertwined with IDD or TBI.
• Responsible to carry out other duties as assigned to support program operations and goals.
