Responsibilities

Position Summary

We are seeking a compassionate, organized, and client-focused Case Manager to join our behavioral health team specializing in substance use disorder and addiction recovery services.

The Case Manager plays an essential role in supporting clients throughout their treatment and recovery by identifying barriers, coordinating services, connecting clients with community resources, and helping clients work toward greater stability and independence.

This position requires strong communication, organization, documentation, crisis intervention, and de-escalation skills, as well as the ability to work collaboratively within a multidisciplinary treatment team in a fast-paced, client-centered environment.

Key Responsibilities

• Providing case management services to clients participating in substance use disorder and behavioral health treatment programs.

• Assessing client needs and identifying barriers related to housing, employment, transportation, healthcare, mental health, legal matters, benefits, education, and other areas that may impact treatment and recovery.

• Developing and maintaining individualized case management/service plans based on identified client needs, treatment goals, and available resources.

• Providing ongoing support and service coordination to assist clients in meeting established goals.

• Connecting and referring clients to appropriate community resources, including housing, employment and vocational services, mental health services, medical care, transportation resources, legal and court-related services, benefits, educational resources, and other community-based supports.

• Coordinating care with therapists, peer support specialists, medical providers, behavioral health technicians, housing staff, and other members of the treatment team to promote continuity of care.

• Utilizing appropriate crisis intervention and de-escalation techniques when working with clients who are emotionally distressed, escalated, or experiencing behavioral concerns.

• Maintaining a calm, professional, and therapeutic approach during challenging or high-stress situations.

• Advocating for client needs while encouraging accountability, independence, self-advocacy, and long-term stability.

• Maintaining professional boundaries and providing services in a respectful, ethical, and client-centered manner.

• Maintaining timely, accurate, complete, and confidential documentation in accordance with agency policies, payer requirements, and applicable state and federal regulations.

• Completing all case management documentation within required agency timeframes.

• Participating in interdisciplinary treatment-team meetings, case reviews, huddles, supervision, and other required meetings.

• Attending all required Case Management meetings, trainings, and professional development activities.

• Maintaining current knowledge of community resources, behavioral health services, substance use treatment resources, and applicable regulatory requirements.

• Following all agency policies, procedures, confidentiality requirements, and professional standards.

• Adhering to the established Case Management KPI structure, including documentation, client engagement, productivity, service completion, and other assigned performance expectations.

• Maintaining required billing productivity and ensuring that services provided meet applicable documentation and medical-necessity requirements.

• Completing other duties as assigned based on program and client needs.

Qualifications

Required

• High school diploma or equivalent.

• Chemical Dependency Counselor Assistant (CDCA) credential.

• Qualified Mental Health Specialist III (QMHS III) qualification or ability to meet and maintain QMHS III requirements as applicable to the position.

• Minimum of one (1) year of relevant experience in behavioral health, substance use disorder treatment, case management, social services, community mental health, or a related field.

• Working knowledge of community resources and social-service systems, including housing, employment, transportation, healthcare, mental health, legal, benefits, and recovery-support resources.

• Strong crisis intervention, conflict-resolution, and de-escalation skills.

• Ability to remain calm and professional while responding to client behavioral concerns, emotional distress, or crisis situations.

• Valid driver's license and reliable transportation.

• Ability to manage multiple client needs and priorities in a fast-paced treatment environment.

Preferred

• Associate's or bachelor's degree in Social Work, Psychology, Behavioral Health, Human Services, Addiction Studies, or a related field.

• Previous experience providing case management services in substance use disorder or behavioral health treatment.

• Strong familiarity with community resources in the Toledo and/or Beachwood, Ohio areas.

• Experience with crisis intervention and behavioral de-escalation.

• Familiarity with electronic health records and behavioral health documentation requirements.
