Job summary

Harmony Health Group in Seymour, TN is seeking a dedicated Case Manager to support the Clinical team in coordinating patient care during treatment. You will document communications, monitor activities, and help manage daily schedules in line with state and federal regulations.

In this role you will perform assessments of clients' needs, maintain progress notes, facilitate cross‑department communication, and assist with discharge planning and linking families to community resources.

Qualifications

• 2+ years of experience with behavioral health needs
• Experience on a multidisciplinary team
• Excellent verbal and written communication skills

Responsibilities

• Assist with supervision of patients' daily activities
• Conduct initial assessments
• Maintain daily progress notes
• Facilitate daily department and cross-department communication
• Document and assign notes
• Manage activity and group notes
• Assist with discharge planning with therapists
• Coordinate with outside providers
• Coordinate benefits documents
• Facilitate family pre-assessments

• Ensure patients follow clinical schedules
