Job summary

Holcomb Behavioral Health Systems – A Chimes Company is seeking a Case Manager to provide community-based services for individuals eligible for behavioral health support. You will assess resources and help clients live safely in their communities, while coordinating with networks of care and funding sources.

The role requires a strong background in case management, a recovery-oriented approach, and the ability to work a 9:00 am–5:00 pm schedule with on-call rotation.

Qualifications

• Bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education.
• Be a registered nurse (RN).
• High school diploma with 12 college credits in social sciences and 2 years of human services with direct client contact.
• High school diploma with 5 years mental health direct care experience in public/private services.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Provide services based on Recovery Model principles.
• Develop individualized, strength-based service plans that are measurable and aim for outcomes.
• Assist consumers in obtaining housing, food, healthcare, employment, and socialization.
• Coordinate with families, payers, and community support services.
