Job summary

CHIMES is seeking a Case Manager to deliver community-based behavioral health services in New Castle. You will assess resources, monitor client stability, and coordinate with a network of providers to support recovery and community living.

The role requires managing up to 30 clients and operating within a Recovery Model framework. The position is Monday–Friday, 9:00 am–5:00 pm, pays $21.00 per hour, and requires relevant education or licensure per the notes.

Qualifications

• Bachelor’s degree in sociology, social work, psychology, or related fields.
• Be a registered nurse (RN).
• High school diploma with social science credits and 2 years in human services with direct client contact.
• High school diploma with 5 years mental health direct care experience.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Provide services based on Recovery Model principles.
• Develop individualized, strength-based service plans with measurable outcomes.
• Assist consumers in obtaining housing, healthcare, employment, and socialization.
• Coordinate with providers to ensure timely service delivery.
• Maintain professional relations with families, payers, and community reps.
• Provide crisis assessment and intervention when needed.
• Submit payroll and billing documentation timely.
• Be available for on-call rotation 24/7.
