Qualifications

• Bachelor’s degree in a human services field preferred or equivalent lived experience navigating behavioral health systems.
• At least 2 years of case management experience preferred.
• Familiarity with navigating Electronic Medical Records (EMR) and Point Click Care platform preferred.
• Knowledge of behavioral health system and local community resources.
• Experience providing brief mental health support over the phone and coordinating care.

Responsibilities

• Assist with crisis evaluation mental health support and discharge follow-up calls.
• Coordinate care by connecting patients to local behavioral health resources and services.
• Coordinate bed searches for psychiatric inpatient admissions and step-down placements.
• Verify insurance and obtain prior authorizations as required.
• Facilitate collateral contacts with families, providers, and community partners.
• Collaborate with ED, inpatient staff, and community partners to ensure continuity of care.
• Maintain accurate EMR documentation throughout referral and admission process.
• Promote teamwork, accountability, and patient-centered care.
• Other duties as assigned.
