Job summary

Harmony Health Group is seeking a Case Manager to support the Clinical team in coordinating patient care within our Massachusetts facilities. You will document communications, monitor activities, and help manage daily patient schedules in accordance with facility policies and state/federal regulations.

The role emphasizes collaboration with therapists to ensure discharge planning, patient communication with outside providers, and coordination of benefits.

Qualifications

• 2+ years of experience working with people with behavioral health needs (trauma, mental illness and/or substance abuse).
• Experience in mental health or Substance Use Disorder services.
• Experience working on a multidisciplinary team.

Responsibilities

• Supervise patients in daily activities and coordinate care with the clinical team.
• Perform initial assessments and document patient needs and plans.
• Maintain daily progress notes and treatment team communications.
• Assist with discharge planning and ensure clear, complete instructions.
