HealthPartners is seeking a Behavioral Health Case Manager to support patients, families, and physicians in addressing behavioral health and social concerns. The role educates and empowers patients to make informed health care decisions and facilitates communication among patient, physician, health plan and community resources.

Qualifications

• Licensed in Minnesota as LICSW, RN or Psychologist; Master’s degree preferred.
• Minimum of 3 years clinical practice experience; 3 years of utilization review or case management experience.
• Demonstrated collaboration, communication, and clinical judgment.

Responsibilities

• Provide support to patients, families, and physicians on behavioral health and social concerns.
• Educate patients and families to enable informed personal health decisions.
• Coordinate care with physicians, health plan and community services.
• Maintain confidentiality per policy and report outcomes to leadership.
• Participate in quality improvement and departmental activities.
