Job summary

HealthPartners is seeking a Behavioral Health Case Manager to support patients, families and physicians in addressing behavioral health and social concerns. The role includes educating patients and families to make informed health care decisions and facilitating communication among patient, physician, health plan and community.

Key responsibilities include coordinating care, maintaining confidentiality, and participating in team-based care to improve member outcomes.

Qualifications

• Licensed in Minnesota as LICSW, RN or Psychologist.
• Demonstrated effective clinical judgement and skills.
• Experience collaborating with care team members and communicating clearly.

Responsibilities

• Provide member-focused support to patients, families, and physicians in addressing behavioral health and social concerns.
• Coordinate service coverage with funding sources when indicated.
• Communicate with patients, families, physicians and health care team members to facilitate collaboration.
• Maintain confidentiality per policies and procedures.
• Participate in department and organizational meetings.
