Job summary

Apicha Community Health Center is seeking a Behavioral Health Case Manager to deliver high-quality behavioral health interventions for patients in the Primary Care Clinic, ensuring access to care and retention.

You will conduct comprehensive assessments, develop individualized care plans, coordinate referrals to specialty care and community services, provide crisis intervention, and collaborate with a multidisciplinary team to improve outcomes for underserved populations.

Qualifications

• Advanced degree in mental health counselling or social work is required.
• Two years of related work experience required; internships/volunteering count.
• LMHC or LMSW licensure required within three months of hire.
• UAS-NY certification required within one month of hire.
• Experience with target populations (SUD, HIV/AIDS, LGBTQ+, etc.) preferred.
• Excellent written and verbal English communication required.
• Spanish or Asian language fluency is a plus.
• Ability to work flexible hours including some evenings and weekends.

Responsibilities

• Provide proactive behavioral health case management with assessments and care plans.
• Provide crisis intervention, stabilization, and brief counseling as needed.
• Coordinate referrals to specialty care, entitlements, and community services.
• Represent Apicha CHC at events to recruit patients, especially low-income/people of color.
• Liaise between patients and multidisciplinary team to coordinate care.
• Respond to phone/email inquiries about services in a timely manner.
• Participate in CQI activities and trainings.
