Mercyhealth is seeking a Behavioral Health Case Manager BS - Outpatient to assess and assist with mental health and educational needs. You will coordinate care, liaise with interagency sources, and act as the primary family contact to improve outcomes. Schedule: 40 hours/week, Days.

The role emphasizes advocacy, collaboration with psychiatrists, and discharge planning to support patients and families throughout treatment and aftercare.

Qualifications

• Bachelor's Degree in mental health or behavioral health field.
• Working toward a higher degree and/or licensure is preferred.

Responsibilities

• Collaborate with physicians and health care staff to understand social and emotional factors affecting the patient.
• Assess participants for continuing treatment needs and develop treatment plans of care.
• Coordinate care with relevant departments and maintain strong interdepartmental communication.
