Job summary

Rosecrance Health Network in Champaign, IL seeks a CCBHC Case Manager to provide mental health rehabilitation services and support adults and families in reducing hospitalization and crises while improving community functioning. You’ll work within the CST delivering case management, counseling, and crisis intervention in a safe, therapeutic setting.

The role requires a Bachelor’s Degree with at least two years in mental health, MHP designation, and strong documentation and organizational skills.

Qualifications

• Bachelor’s Degree with minimum two years’ experience in mental health.
• Demonstrated education, training or experience in the mental health services.
• MHP designation.
• Adequate written skills to accurately complete required documentation within the time frames.
• Excellent organizational and computer skills.
• Self-starter with ability to work independently and with a team.
• Skilled in conducting individual, family and group counseling, case management.
• Ability to present and coordinate lectures for clients, families, staff or community resources.
• Valid driver’s license.

Responsibilities

• Work hours prescribed by and any additional hours deemed necessary for the responsibilities assigned.
• Motivate and problem solve with staff regarding effective clinical practices.
• Be self-motivated, seek improved performance, problem solving while meeting client needs.
• Adapt verbal and written communication to client and family.
• Complete all necessary documentation within the required time lines.
• Undertake primary case management responsibilities for designated clients.
• Conduct individual, group, and family counseling and educational sessions for clients and/or family members.
• Ensure client service records are maintained per regulations.
• Maintain communications with all staff (admissions, process, discharge plans).
• Coordinate services from community resources.
• Present workshops, lectures and training to clients, families, staff and community groups.
• Be familiar with court proceedings and available to make court appearances on behalf of Rosecrance clients.
• Serve as a member of the Clinical Team and participate in all team meetings.
• Exercise confidentiality in line with professional conduct and law.
• Deliver exceptional customer service consistently to every customer.
• Serve as a role model and demonstrate positive guest relations.
• Assume other related responsibilities as delegated by management.
