Job summary

Rosecrance is seeking a Case Manager for Assertive Community Treatment in Champaign, IL. This role provides mental health rehabilitation services, supports adults and families, and aims to reduce hospitalizations and crises while enhancing community functioning.

Qualifications

• Bachelor degree or equivalent experience in a related field.
• Must qualify as an MHP per Illinois DMH Rule 132.
• Strong written and oral communication skills.
• Ability to quickly establish rapport with clients and families.
• Computer proficiency and documentation in the electronic health record.
• Ability to complete documentation within prescribed time frames.

Responsibilities

• Work scheduled hours and any additional hours as needed by leadership.
• Motivate and problem-solve with staff on clinical practices.
• Be self-motivated and seek performance improvement.
• Adapt communication to client/family language and style.
• Complete required documentation on time.
• Undertake primary case management for designated clients.
• Provide crisis intervention and medication monitoring as needed.
• Conduct individual, group, and family sessions.
• Maintain client service records per regulations.
• Coordinate services with community resources.
• Participate in on-call rotations.
• Present workshops and trainings to clients and staff.

Skills

Bachelor-level degree in related field
