Job summary

Rosecrance, Inc. is seeking a Case Manager for Assertive Community Treatment in Champaign, IL. This on-site clinical role supports adults and families, delivers mental health rehabilitation, and aims to reduce crises and hospitalization while improving community functioning.

Requires a Bachelor’s in psychology/counseling/social work or related field, and MHP qualification per IL Rule 132. Competitive pay and benefits offered.

Qualifications

• Bachelor-level education or equivalent experience in psychology, counseling, social work, or related field.
• Must qualify as a Mental Health Professional (MHP) per IL Rule 132.
• Excellent written and oral communication skills.
• Ability to establish rapport quickly.
• Proficient with computers and EHR documentation.
• Experience with individual, family and group counseling.

Responsibilities

• Work scheduled hours and additional hours as required by leadership.
• Provide crisis intervention and medication monitoring as needed.
• Conduct individual, group, and family sessions.
• Maintain client records per regulations and timelines.
• Coordinate services with community resources.
