Job summary

You will assess client needs, develop individualized service plans, and collaborate with multidisciplinary teams to ensure effective and client-centered services.

The role emphasizes advocacy, reviewing case progress, and supporting clients toward independence while adhering to CARF and CMS documentation practices.

Qualifications

• Strong interpersonal communication with clients, families and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.
• CPR certification preferred.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and families to meet behavioral health needs.
• Facilitate clients in achieving independence and daily living skills.
• Perform other duties as assigned.
• Uphold documentation and billing practices associated with CARF, CMS.
