Job summary

Qualifications

• High school diploma or equivalent.
• 36 months of behavioral health experience or 60 college credit hours in any field AND 12 months of behavioral health experience.
• Bachelor's or Master's degree in any field with 6 months behavioral health experience.
• Maintain current case management certification.
• Basic understanding of social and cultural factors influencing human behavior.
• Capability to work as part of a team.
• Core competencies in relation to individuals with co-occurring disorders.
• Case Management certification.

Responsibilities

• Provide advocacy, linkage, and referral services on behalf of chronically mentally ill.
• Provide intake, screening, referral and case management services.
• Assist in maintaining an active liaison with community agencies serving the “high risk” population with modified PACT program.
• Maintain the clinical records system in accordance with ODMHSAS, OHCA, CARF and CREOKS rules and regulations.
• Attend and regularly present cases in the treatment team meetings.
• To read and be knowledgeable of all current CREOKS policies and procedures.
• Maintain productivity averages of direct service to clientele as set by CREOKS for full time employment.
• Coordinate and manage indigent medication/patient assistance program.
• Turn in billing and associated documentation as mandated by CREOKS policy.
• Responsible for understanding and following all policies and procedures of the agency.
• Responsible for providing Psychiatric Social Rehabilitation Services as assigned by Supervisor.
• Responsible for attending and presenting cases in treatment team staffing conferences.
• Maintain full time caseload as assigned by Site Director and meeting daily productivity averages.
• Billing documentation and daily clinical documentation completion.
• Interact with client families and other agencies to ensure appropriate therapeutic treatment.
• Participate in mandatory trainings such as CAPE and CPR.
• Implement rehabilitative curriculum and develop support materials based on best practices and evidence-based treatment models.
• Teach Rehabilitative skills for daily living and community integration.
