Job summary

Anew Behavioral Health is seeking a Behavioral Health Case Manager to coordinate comprehensive care for clients with mental health and substance use challenges. The role emphasizes developing individualized service plans and collaborating with a multidisciplinary team to promote recovery and independence.

Responsibilities include managing case assignments, drafting service plans, advocating for clients, and ensuring safe, effective, and client-centered care.

Qualifications

• CDCA/Case Management certification required.
• CPR certification preferred.
• NPI number/Ohio Medicaid ID required (instructions on how to apply).
• Minimum of 3 years in behavioral health or related field preferred.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and family members to ensure behavioral health needs are met.
• Facilitate clients in achieving independence and developing daily living skills.
• Perform other duties as assigned.
