Qualifications

• Master’s Degree accredited by the Accreditation of Counseling and Related Educational Programs (e.g., social work, clinical mental health counseling).
• Minimum of 1 year of experience.
• Preferred: Experience in a healthcare setting.

Responsibilities

• Conduct initial screenings and psychosocial assessments using standard tools and guidelines.
• Provide brief therapeutic interventions, crisis intervention, and support for grief, trauma, or life transitions.
• Coordinate care, follow-up appointments, and referrals to community resources as needed.
• Collaborate with primary care teams and participate in case reviews and team huddles.
