The Department of Health and Social Services (DHSS) seeks a professional to assist with intake, assessment, and case management for Mental Health court clients. The incumbent will work with clients in the community and DOC, coordinating referrals and monitoring progress to support court outcomes.

Responsibilities include documenting assessments, preparing reports, and collaborating with a multi-disciplinary team to determine appropriate service placements and support options.

Qualifikationen

• Bachelor's degree in Behavioral or Social Science or related field.
• Six months' experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
• Six months' experience in providing clinical treatment, counseling or determining eligibility for services or benefits as part of a treatment team.
• Six months' experience in interpreting laws, rules, regulations, standards, policies, and procedures.
• Six months' experience in narrative report writing.

Aufgaben

• Interviews clients, family members or other persons to obtain complete case history.
• Provides guidance to the client's family to enable solutions and promote positive interactions.
• Evaluates information and refers to proper channels for review.
• Prepares narrative, statistical and court reports as necessary.
• Participates in multi-disciplinary teams to assess client needs and plan treatment.
