Job summary

The Delaware Department of Health and Social Services (DHSS) seeks a Behavioral Health Case Manager II to assist intake, assessment, and case management for Mental Health court clients in community and DOC settings. You will coordinate referrals and prepare court reports to support client placement and services.

Qualifications

• Bachelor's degree or higher in Behavioral or Social Science or related field.
• Six months' experience in case management with assessment, planning and service coordination.
• Six months' experience in narrative report writing and documenting client progress.

Responsibilities

• Assists with intake, referral, assessment and case management for Mental Health court clients.
• Interviews clients and family members to collect case history and needs.
• Refers clients to appropriate care levels and coordinates referrals.
• Prepares court reports and narrative/statistical documentation as required.
• Coordinates services to support TASC clients and related stakeholders.
