Orlando Health Physician Associates is seeking a Case Manager to support adult and adolescent patients. The role involves evaluating complex needs, coordinating referrals, and advocating for care across our Central Florida network.

The position requires a Bachelor’s degree in psychology, social work, or a healthcare field, at least one year of case management experience, and a Florida driver’s license. Travel up to 5% and a potential hybrid schedule are expected.

Competenze

• Bachelor’s degree in psychology, social work, or healthcare related field.
• One year of experience in case management or related patient care support in a community agency, outpatient facility, or hospital setting.
• Valid Florida driver’s license with no more than six points, at least 21 years old, and insurable under Orlando Health carrier’s requirements.

Mansioni

• Assess adult and adolescent clients within assigned population, including diagnosis, history, current treatments, age, payment source, and needs.
• Obtain referrals from behavioral health and other providers to assess client needs.
• Act as an advocate for patient health care needs.
• Provide outreach and education at health fairs and community events.
• Facilitate family meetings to discuss lab results, recommendations, and care plans.
• Maintain organized records and documentation of work performed.
• Maintain professional relationships with supervisor, peers, and families.
• Ensure attendance and compliance with Orlando Health policies.
• Maintain confidentiality in compliance with HIPAA and policies.
• Complete orientation and department competencies.
