Qualifications

• Bachelor’s degree required in psychology, social work, or healthcare field.
• At least 1 year of case management or related patient care experience.
• Valid Florida driver's license with no more than six points, at least 21 years of age, and insurable per company requirements.

Responsabilités

• Assess adult and adolescent clients across diagnosis, history, treatments, age, payment source, and care needs.
• Obtain referrals from Behavioral Health and other sources to assess client needs.
• Advocate for patient health care needs and coordinate care across teams.
