Job summary

Community Bridges is seeking a dedicated Case Manager to provide community and office-based services. You will coordinate care, connect patients with health services, financial aid, employment, and social supports while monitoring progress and documenting milestones.

The role emphasizes recovery-oriented practice, collaboration with care teams, and ensuring access to needed services to improve patient outcomes and quality of life.

Qualifications

• Outreach and engagement with patients and families in behavioral health
• Basic understanding of GMH/SU population needs and services
• Recovery-oriented approach and ability to navigate health services

Responsibilities

• Provide case management services in community and/or office settings
• Assist patients in accessing health services, financial aid, employment, education, and social supports
• Assess needs, develop individualized treatment plans, monitor progress, and document services
• Support recovery and create a safe, supportive environment for clients
