Bridges Healthcare, Inc. is seeking a Case Manager in our Community Support Program. The position is full time at 35 hours per week, with an hourly wage of $22.46–$28.87 depending on experience.

Qualifications

• Bachelor's degree required, human services preferred.
• Flex schedule required as needed.
• Knowledge of behavioral health issues and community integration.
• Strong communication and documentation skills.
• Computer literacy.
• Valid driver's license and reliable transportation.
• CPR/First Aid certification or willingness to obtain.

Responsibilities

• Facilitate recovery-focused activities to motivate consumers toward improved quality of life.
• Assist consumers in crisis planning and advanced directives for emergencies.
• Provide advocacy and education on treatment barriers and stigma.
• Network and collaborate with community resources and providers.
• Assist in the development of consumer's treatment plan.
• Maintain client records.
• Assist in the development of program and client budgets.
• Supervise, coach and provide guidance to supervisees.
• Perform other department or agency related duties as directed by supervisor.
