Job summary

Milestone Treatment Center is seeking a compassionate, organized Case Manager to support individuals in treatment for substance use and mental health conditions. You will help clients identify needs, connect them with resources, and remove barriers to recovery.

The role emphasizes advocacy, coordination with a multidisciplinary team, and strong documentation practices to support long-term stability in the community.

Qualifications

• Experience in case management, social work, behavioral health, substance use treatment, mental health, crisis intervention, or human services.
• Active or pending LCADC or QMHP-A credential preferred.
• Experience connecting individuals with community resources and support services.
• Strong interpersonal and relationship-building skills.
• Excellent verbal and written communication skills.
• Ability to advocate effectively while maintaining appropriate professional boundaries.
• Strong organizational skills and attention to detail.
• Ability to manage multiple clients, priorities, and follow-up activities.
• Comfortable working independently while contributing to a multidisciplinary team.
• Ability to remain calm and solution-focused when clients encounter difficult situations.
• Proficiency with Google Workspace and comfort learning electronic documentation systems.

Responsibilities

• Build strong, professional relationships with clients and serve as a consistent source of support, guidance, and advocacy.
• Assess each client's individual needs, challenges, strengths, and available resources.
• Develop, implement, and monitor individualized case management plans.
• Connect clients with community resources related to housing, employment, healthcare, behavioral health, transportation, benefits, and other support services.
• Help clients navigate complex systems and processes, including housing programs, government assistance, insurance, and community services.
• Assist with verification of benefits and related coordination when required.
• Provide support and appropriate intervention during challenging or crisis situations.
• Facilitate groups or educational sessions as needed.
• Advocate for clients and help eliminate barriers that could interfere with treatment, recovery, and successful transition back into the community.
• Coordinate with outside providers, agencies, families, and community organizations when appropriate.
• Collaborate closely with therapists, medical providers, counselors, and other members of our multidisciplinary treatment team.
• Maintain timely and accurate case notes, documentation, referrals, and client records.
• Track client progress and follow through on outstanding needs and resources.
• Maintain appropriate professional boundaries and client confidentiality.
• Follow applicable HIPAA, 42 CFR Part 2, regulatory, ethical, and Milestone requirements.
