Job summary

Milestone Treatment Center is seeking a compassionate Case Manager to support individuals in treatment for substance use and mental health conditions. The role focuses on coordinating housing, employment, insurance, transportation, healthcare, benefits, and other real-world needs.

You’ll identify needs, connect clients with resources, and remove barriers to treatment and long-term stability, advocating for clients while maintaining professional boundaries.

Qualifications

• Experience in case management or social services in behavioral health settings.
• Credential preferred: LCADC or QMHP-A.
• Experience linking clients to community resources and support services.
• Excellent interpersonal and written communication skills.
• Ability to advocate while maintaining professional boundaries.

Responsibilities

• Build relationships with clients and provide consistent support and advocacy.
• Assess client needs, challenges, strengths, and resources.
• Develop, implement, and monitor individualized case management plans.
• Connect clients with housing, employment, healthcare, and other services.
• Navigate housing programs, government assistance, insurance, and community services.
• Maintain case notes, referrals, and client records.
• Collaborate with therapists and multidisciplinary team.
