Job summary

Anew Behavioral Health in Mount Vernon, Ohio is seeking a Behavioral Health Case Manager to provide comprehensive support and coordination of care for clients facing mental health and substance use challenges.

You will assess client needs, develop individualized service plans, and collaborate with multidisciplinary teams to ensure effective, client-centered services that promote recovery, independence, and wellbeing.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in Human Services preferred.
• CPR certification preferred; NPI number/Ohio Medicaid ID required.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving independence and developing daily living skills.
• Perform other duties as assigned.
• Upholds documentation and billing practices compliant with CARF and CMS.
