Job summary

SEARHC seeks a Behavioral Health Case Manager to support adults and families facing serious emotional, mental, and substance use disorders. You will manage a caseload, coordinate with health and social services, document care, and advocate for clients to achieve stabilization and recovery.

The role emphasizes outreach, collaboration with an integrated care team, and maintaining competencies through mandatory trainings.

Qualifications

• Bachelor’s degree in social services or related field preferred.
• Equivalent combination of education and experience may substitute for the degree.
• Alaska Native Tribal Health Consortium Behavioral Health Aide II certification or PQAP eligibility.

Responsibilities

• Manages and monitors assigned client caseload to support independence and recovery.
• Links clients to health and social services and coordinates care.
• Documents services and obtains prior authorizations.
• Responds to urgent client circumstances as needed.
• Coordinates referrals with Referral Care Coordinators.
• Provides outreach and advocacy in the community.
• Attends trainings and maintains annual competencies.
