Responsibilities

• Manages assigned client caseload to support independence, safety, and stability.
• Assists clients to link and coordinate with health and social services.
• Provides supportive active listening to engage and motivate clients toward self-directed change.
• Documents services in EHR and handles prior authorizations.
• Responds promptly to urgent client circumstances.
• Engages in outreach and advocacy in the community.
