Job summary

UofL Health is seeking a case manager to collaborate with therapists and the interdisciplinary team to prioritize biopsychosocial needs and discharge planning resources. You will document interventions on Treatment Plans and coordinate with community resources.

Responsibilities include discharge coordination, maintaining resource materials, leading psycho-educational activities, and advocating for patient rights while ensuring HIPAA compliance and quality care.

Qualifications

• Bachelor's degree in human services or related field (required)
• Master's degree in human services or related field (preferred)
• At least 18 months of experience providing substance use, mental health, or co-occurring treatment services in a behavioral health or related human services setting (required)
• Three (3) years' experience (preferred)
• Basic Life Support (BLS) certification (required)

Responsibilities

• Identifies patient biopsychosocial needs and connects patient with community resources
• Interacts with patient and guardians for discharge planning per Treatment Plan goals
• Coordinates referrals and discharge appointments with other service providers
• Maintains discharge plans and collaboration with community resources and patients
• Maintains an up-to-date resource center of community resources for the team
• Provides psycho-educational groups to patients as needed
• Advocates for patient rights at all times
• Assists with patient satisfaction surveys near end of treatment
