Job summary

UofL Health is seeking a case manager to collaborate with therapists and the treatment team to prioritize patients’ biopsychosocial needs and discharge planning, documenting individualized interventions on Treatment Plans. You will connect patients with community resources, coordinate referrals, and maintain discharge documentation while supporting a therapeutic milieu and protecting patient rights.

Education includes a Bachelor’s degree in human services or related field (Master’s preferred);

Qualifications

• Bachelor’s degree in the human services or related field is required.
• Master’s degree in the human services or related field is preferred.
• Minimum 18 months of experience in behavioral health or related human services setting is required.
• Basic Life Support (BLS) accreditation is required.

Responsibilities

• Identify patient biopsychosocial needs and connect with community resources.
• Assist in discharge planning per Treatment Plan goals and interventions.
• Coordinate referrals and discharge appointments with care teams.
• Maintain accurate discharge plans and collaboration records.
• Support therapeutic milieu and psycho-educational group activities as needed.
• Act as a patient advocate and uphold patient rights at all times.
• Assist with patient satisfaction surveys near discharge.
