Job summary

UofL Health in Louisville, KY, is seeking a Case Manager to collaborate with therapists and an inter-disciplinary team to prioritize patients' biopsychosocial needs and discharge planning resources.

You will document interventions on Treatment Plans, connect patients with community resources, coordinate referrals, and support a therapeutic milieu while protecting privacy and confidentiality.

Qualifications

• Bachelor’s degree in the human services or related field (required)
• Master’s degree in the human services or related field (preferred)
• Basic Life Support (BLS) certification (required)

Responsibilities

• Identifies patient biopsychosocial needs and connects patient with appropriate community resources
• Interacts directly with patient and guardians to assist in discharge planning per Treatment Plan goals/interventions
• Coordinates referrals and discharge appointments and collaborates with other service providers and community resources
• Maintains accurate documents of discharge plans and collaboration with community resources, patients and others involved in patient’s care
• Maintains an up-to-date resource center of community-based resources for clinical team to utilize for patients/family’s needs
• Assists in therapeutic milieu to provide structured interventions and psycho-educational groups to patients
