Qualifications

• Bachelor’s Degree in a behavioral health field or related area.
• 1+ year of related Behavioral Health experience preferred.
• Previous case management experience preferred.

Responsibilities

• Develop and implement the patient service plan (ISP).
• Coordinate discharge planning and aftercare options.
• Maintain comprehensive clinical records and coordinate referrals.
• Collaborate with treatment team and family for care plans.
• Coordinate referrals for services, housing, employment, etc.
