Qualifications

• Minimum High School Diploma or Equivalent.
• Bachelor's Degree preferably in Psychology or related field.
• Current CPR certification required.

Responsibilities

• Oversee patient admissions and services, including creating and organizing all relevant medical and legal documents (creating/maintaining charts).
• Gather information to document history and identify primary issues for treatment and obtain collateral information from patient and significant others to transcribe relevant information.
• Develop and coordinate individualized discharge plans from admission by utilizing treatment team and chart information to determine aftercare needs. Communicate with family or community resources to provide an aftercare plan.
