Bakersfield Behavioral Healthcare Hospital in Bakersfield, CA is seeking a Case Manager to oversee admissions and service coordination in a fast-paced behavioral health setting. You will document history, identify primary issues for treatment, and coordinate discharge planning with the treatment team.

Responsibilities include communicating with families and community resources to arrange aftercare, maintaining charts and relevant records, and ensuring seamless care transitions for patients

Qualifications

• High School Diploma or equivalent is required; a Bachelor's in Psychology or related field is preferred.
• CPR certification is required or must be obtained before start date.
• Experience in a mental healthcare hospital environment is preferred.
• Strong communication, empathy and teamwork are essential.

Responsibilities

• Oversee patient admissions and services, including creating and organizing medical and legal documents.
• Gather information to document history and identify issues for treatment; obtain collateral information.
• Develop and coordinate individualized discharge plans from admission with the treatment team.
• Communicate with families and community resources to arrange aftercare.

Education
