• Develop and coordinate individualized discharge plans from admission by utilizing treatment team and chart information to determine aftercare needs. Communicate with family or community resources to provide an aftercare plan.

Job Summary

We are looking for a Case Manager to help with BBHH's vision of providing excellent care in collaboration with our Inpatient Units, striving to be the best hospital we can be.

The Case Manager will oversee patient admissions and services, including creating and organizing all relevant medical and legal documents (creating/maintaining charts).

The Case Manager will gather information to document history and identify primary issues for treatment and obtain collateral information from the patient and significant others/support people to transcriberelevant information.

Develops and coordinates individualized discharge plans from admission by utilizing treatment team and written chart information to determine the patient's aftercare needs. Communicates by phone, fax or other correspondencewith family or community resources such as nursing homes, group homes, or mental health providers to provide an aftercare plan.

Though we do need to meet some minimum requirements for the position such a High School Diploma or Equivalent, and a Bachelor's Degree, preferably in Psychology or a related field, we're really looking for people who bring their HEART to work.

Additionally, you'll complete a "Handle-With-Care" Physical Restraint Technique Training during your orientation so that you know how and when to engage when such an event occurs.

The Case Manager position pays between $23.00 to $29.00 per hour based on the experience you bring with you.
