Job summary

Integrated Services of Kalamazoo (ISK) is seeking a full-time Case Manager to join our Community Treatment Team (CTT). You will provide linking, advocacy, coordination and monitoring for adults with serious mental illness and/or substance use disorders in our program.

You will work with ISK, Kalamazoo Court System, DHHS and local hospitals to connect clients with needed services, while performing outreach and timely clinical documentation.

Qualifications

• Minimum of one year of experience working with adults with serious mental illness, substance use disorders or co-occurring conditions.
• Bachelor's degree in Psychology, Social Work, or a related field.
• A valid Michigan driver's license and reliable transportation are required.

Responsibilities

• Provide linking, advocacy, coordinating and monitoring for adults with a serious mental illness and/or substance use disorders in the case management program.
• Collaborate with public service systems (ISK, Kalamazoo Court System, DHHS, hospitals) to coordinate services.
• Provide outreach, assess safety, and complete clinical documentation accurately and timely.
