The West Michigan Hispanic Chamber of Commerce seeks a Case Manager – Specialized Care & Emergency Services to join our team in Kalamazoo. This full-time, day-shift role starts at $50,440 per year and offers opportunities to support individuals in behavioral health crises through collaborative, trauma-informed care.

If so, we have an exciting opportunity for you to work as a full-time (40hrs/wk) Case Manager – Specialized Care & Emergency Services!
What You Will Be Doing Working as an Case Manager – Specialized Care & Emergency Services you will work as a member of our ICS team to provide short term crisis services to individuals in our community. This position will provide collaborative, strengths based, trauma informed care to adults and/or youth and their families who are experiencing a mental health crisis. You will assist individuals and families by evaluating, providing support and connecting to resources and services in the community in collaboration with community providers and resources in order to assess and/or provide the appropriate level of care.
What We Require You will have a bachelor’s degree in social work with a minimum of one years’ experience working with adults with serious mental illness, substance abuse and/or co-occurring disorders that also have co-morbid health conditions. You will also have experience working with youth with serious emotional disturbances and co-occurring disorders. Individuals with a BSW will provide crisis intervention services, connect individuals with resources and complete safety planning with at risk individuals. A valid Michigan Motor Vehicle Operator's license is required. Required License: LBSW, or LLBSW.
