Job summary

Crusader Community Health in Rockford, IL is seeking a Behavioral Health Case Manager to deliver direct services in the clinic and community within the Integrated Primary Care model.

The role includes comprehensive case management, behavioral health counseling, education, skills training for patients and families, health promotion, and coordinated referrals to community services.

Bachelor’s degree required; CADC preferred; experience with substance use and mental health; EHR experience preferred.

Qualifications

• Bachelor’s degree in human services is required.
• Master’s degree in Social Work/Counseling/Psychology or related field is preferred.
• CADC certification or completion of CADC certification is preferred.
• Experience with substance use disorders is required.
• Experience with mental health issues is required.
• Case management/care management experience is preferred.
• Electronic Health Record experience is preferred.

Responsibilities

• Provide services in clinic and community within Integrated Primary Care model.
• Deliver comprehensive case management and behavioral health counseling/education.
• Provide patient and family skills training and support.
• Promote health and coordinate transitional care.
• Advocate, coordinate and refer to community services and agencies.
• Collaborate with providers, patients and families to develop care plans.
