Responsibilities

  • Coordinate patient care with primary care and behavioral health teams.
  • Conduct behavioral health assessments and care plans.
  • Maintain outreach and follow-up with high-risk patients.
  • Provide psychoeducation to patients and families.
  • Connect patients with community resources and social services.
  • Coordinate referrals, authorizations, and follow-ups.
  • Assist patients in goal setting and action planning.
  • Document within the Electronic Health Record (EHR).
  • Participate in trainings, meetings, and quality initiatives.
  • Ensure compliance with HIPAA and organizational policies.
  • Perform other duties as assigned.

Job description

Position Summary

TheBehavioral Case Manager is responsible for providing comprehensive carecoordination and case management services to patients receiving behavioralhealth services at Health Care Partners of South Carolina. This positionsupports individuals experiencing mental health and substance use challenges byassessing patient needs, including Social Determinants of Health (SDOH),coordinating services, facilitating appointments, connecting patients withcommunity resources, and promoting treatment adherence. Working collaborativelywith behavioral health providers, primary care providers, interdisciplinarycare teams, and community partners, the Behavioral Case Manager helps developindividualized care plans, reduce barriers to care, and improve overall healthoutcomes through integrated, patient-centered care.

Essential Duties and Responsibilities
  • Collaborate with primary care providers,behavioral health clinicians, and interdisciplinary care teams to coordinatepatient care.
  • Complete behavioral health assessments, safetyplans, recovery plans, and other care management documentation as appropriate.
  • Maintain regular outreach and follow-up withpatients, including those identified as high risk for substance use,suicidality, or self-harm.
  • Provide psychoeducation to patients and familiesregarding behavioral health diagnoses, treatment options, medications, andrecovery resources.
  • Connect patients with community-based resourcesand assist with accessing social services, financial assistance, housing,transportation, and other support services as appropriate.
  • Coordinate referrals, prior authorizations, andfollow-up appointments with internal providers and external community partners.
  • Assist patients in identifying goals, developingaction plans, and overcoming barriers to treatment and recovery.
  • Participate in ongoing communication with casemanagement staff, utilization management staff, health plan partners, andcommunity providers to support coordinated care.
  • Maintain accurate, timely documentation withinthe Electronic Health Record (EHR).
  • Assist with the implementation of organizationalpolicies, procedures, and quality improvement initiatives related to casemanagement and behavioral health services.
  • Ensure compliance with HIPAA, organizationalpolicies, and applicable state and federal regulations.
  • Participate in required trainings, staffmeetings, and continuing education activities.
  • Perform other duties as assigned.
Education & Experience
  • Bachelor's degree in Social Work, Psychology,Human Services, Counseling, or a related behavioral health field required.Master's degree preferred.
  • Minimum of one (1) year of experience in casemanagement, behavioral health, counseling, healthcare, social services, or arelated human services field required.
  • Experience working with individuals experiencingmental health and/or substance use disorders preferred.
  • Experience working in a Federally QualifiedHealth Center (FQHC), community mental health setting, integrated primary caresetting, or similar healthcare environment preferred.
  • Certified Case Manager (CCM) credential is aplus but not required.
Knowledge, Skills, and Abilities
  • Effective communication and interpersonal skillswith the ability to establish trust and rapport with patients from diversebackgrounds.
  • Strong empathy, active listening, and patientadvocacy skills.
  • Knowledge of behavioral health conditions,co-occurring disorders, and substance use disorders.
  • Familiarity with community resources, supportservices, and referral networks available to assist patients with healthcareand social needs.
  • Knowledge of Social Determinants of Health(SDOH) and their impact on patient outcomes.
  • Strong organizational and time management skillswith the ability to prioritize multiple responsibilities in a fast-pacedhealthcare environment.
  • Ability to collaborate effectively withmultidisciplinary healthcare teams and community partners.
  • Proficiency in Microsoft Office and ElectronicHealth Record (EHR) systems.
  • Knowledge of HIPAA regulations and the abilityto maintain the confidentiality, security, and integrity of patientinformation.
