Firelands Health in Sandusky County, OH seeks a Behavioral Health Case Manager to deliver community-based services to youth and adults with mental health and substance use disorders under supervision of the Program Coordinator. The role includes intake assessments, treatment planning, crisis support, day treatment, safety planning, and thorough documentation.

Emphasis on recovery, collaboration with the treatment team, and compliance with Medicaid/Medicare and accrediting standards.

Qualifications

• Associate’s degree or higher in social services related field preferred.
• Heart Saver, Aegis Crisis Prevention, and First Aid certifications within 6 months of hire.
• Ability to effectively understand and communicate in written and oral form; respect for cultural needs.
• Punctual attendance at assigned work location is required.
• Ability to be mobile with a valid Ohio driver's license and be insurable under Firelands’ auto policy.
• Ability to work safely and follow OSHA/CDC precautions; complete probationary period.

Responsibilities

• Provides community-based services to youth and adults with mental health and substance use disorders under supervision.
• Performs ongoing assessment of needs and coordinates the ITP with services identified.
• Provides crisis support and stabilization, including safety planning and afterhours coordination.
• Delivers Day Treatment services as assigned by Program Coordinator.
• Ensures services comply with Medicaid/Medicare, HFAP, OhioMHAS, and CARF.
