Firelands Health is seeking a BH Case Manager in Sandusky County to provide community-based case management for youth and adults with mental health and substance use disorders. The role includes ongoing assessment, service coordination, crisis support, and collaboration with the treatment team.

Qualifications

• Associate’s degree or higher in social services related field preferred.
• Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
• Ability to effectively understand and communicate; good written and spoken communication.
• Must be punctual and able to work at the assigned location; valid Ohio driver’s license and insurance.
• Ability to work safely, comply with OSHA, CDC guidelines, and maintain accident-free workplace.
• Successful completion of a 180‑day probationary period.

Responsibilities

• Under supervision, provide community-based services to youth and adults with mental health and substance use disorders.
• Deliver case management services: ongoing needs assessment, linkage with supports, symptom monitoring, advocacy, and coordinated services.
• Provide crisis support and stabilization, assist with hospitalizations when indicated, and coordinate with the treatment team.
