Job summary

Elk Ridge Community Health seeks a Behavioral Health Case Manager in Flagstaff, AZ to join the patient care team. Schedule: Monday–Friday.

You will provide direct case management, coordinate with the interdisciplinary team, and advocate for patients and families to ensure timely, quality behavioral health services. You will document patient care per regulatory standards, support transitions to community resources, and maintain HIPAA-compliant records while pursuing improved outcomes and patient

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Sociology, Public Health or related field.
• At least 1 year of experience in human services or behavioral health/case management.
• Current BLS certification.
• Level I fingerprint clearance card.
• Bilingual in English/Spanish is preferred.

Responsibilities

• Provides supportive case management to patients actively enrolled in behavioral health services.
• Coordinates care planning with the interdisciplinary team to ensure continuity of care.
• Ensures documentation meets HIPAA and regulatory standards.
• Facilitates referrals to ACA Marketplace, AHCCCS, or discount programs.
• Monitors patient progress, follows up after crises or missed appointments, and supports transitions to community resources.
