Job summary

Shasta Community Health Center is seeking an IBH Case Manager to provide case management for patients with mental health and substance use concerns, linking them to clinic and community resources. You will evaluate needs, contribute to treatment plans, and document progress for the care team.

You will coordinate services, accompany patients to access resources, and maintain records per regulatory standards, while collaborating with multidisciplinary teams to improve health outcomes.

Qualifications

• Knowledge of adult learning theories and group dynamics.
• Knowledge of various community resources in Shasta County area.
• Ability to work autonomously and seek consultation when needed.
• Participates in planning and evaluation of case management program.
• Excellent verbal and written communication skills.
• Typing 35 WPM or higher.

Responsibilities

• Accept newly assigned cases and complete initial assessments per protocol.
• Develop service plans with patients and educate to make informed choices.
• Collaborate with treatment teams to support patients' goals.
• Coordinate and monitor services from appropriate resources.
• Document contacts and progress in the electronic health record.
• Participate in interdisciplinary team meetings and education activities.
