Qualifications

Responsibilities

• Identify, implement, maintain, monitor, and modify behavioral health services as needed.
• Coordinate care with patient, HCDM, providers, family, resources, and community supports.
• Assist patients in applying for social security benefits.
• Outreach and follow-up after crisis events and engage in case staffing.
