Qualifications

• High school diploma or GED required; Associates degree in a related field preferred.
• 1-3 years of GMH/SU experience required.
• 1-3 years of outreach/engagement experience in behavioral health.
• 1-3 years of medical setting experience preferred.
• Minimum 6 months recovery from substance use and/or mental health disorders preferred.
• BHT certification as per A.A.C. R9-10-101.33.

Responsibilities

• Provide case management in community or office settings to patients and families.
• Plan, connect, advocate, coordinate and monitor patient needs and progress.
• Assist patients in gaining access to health services, financial aid, employment and social supports.
