Job summary

ViaQuest Psychiatric & Behavioral Solutions in Wooster, OH seeks a Behavioral Health Case Manager to support children, adolescents and adults through therapeutic behavioral services, care coordination and individualized treatment planning.

This full-time, Monday–Friday 9am–5pm role offers a starting wage of $18–$21 per hour based on education and experience, plus a monthly incentive program for exceeding productivity expectations and a competitive benefits package.

Qualifications

• HS diploma plus three or more years of relevant field experience or a Bachelor's or Master's degree in a related field.
• Related experience or training with mental or behavioral health conditions.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordination of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
